Dr. Gutow provides personalized services tailored to each family and situation and believes she has greater flexibility to meet the needs of families when not under the direction of managed care. Her practice is fee-for-service which allows for more privacy and responsiveness in the assessment process. (please see here as to the difference between the services offered at Great Minds of New Mexico and those provided via the public schools).
Although Dr. Gutow is not a provider on any insurance panels and does not bill insurance directly, she can provide documentation to be submitted to your insurance plan for reimbursement. Insurance plans vary widely with regard to the coverage they offer for assessment services, and it is recommended that you contact your insurance company directly to clarify the coverage you have, and the process to receive benefits. Some insurance companies require pre-authorization, so please verify with your insurance provider if this is a requirement and allow adequate time for forms to be completed prior to testing. Some families also have access to Health Savings Accounts or Flexible Spending Accounts, which may provide reimbursement. Again, this is individual to your plan and it will be important to familiarize yourself with your plan’s requirements and coverage.

Dr. Gutow does not participate with any insurance plans.
Payment is expected at the time of service as outlined in the Service Agreement and may be made by check, money order, cash, or credit card (Mastercard or Visa). All fees will be charge the Returned checks or charges will incur an additional $25.00 fee, and ongoing unpaid balances may incur interest and/or collections charges. A minimum payment of 50% of the assessment fee is due at first testing session; the balance is due at the second testing session. If the child attends a meeting without a parent, payment will need to either be sent with the child or paid prior to the meeting. The report will not be made available until all fees are paid in full. All fees are reviewed annually and may be subject to change. Please note that all services are taxed at the state and local rates as well.
The initial intake meeting (60-minute clinical interview) is billed as a flat fee of $100.
Assessments are billed as a flat fee ranging from $700-$2000, depending on depth of the testing required to answer the referral question (includes 2-to-3 testing sessions and a feedback meeting).
Consultation services* (i.e., in-office consultation, school meetings, or on-site consultation services): $100 per 60 minute session plus travel of $.36 per mile, when relevant.

Dr. Gutow requires a minimum of 24 hours advance notice for cancelled appointments. If a minimum of 24 hours notice is not provided, a cancellation fee of $100 may be charged as outlined in the Service Agreement.
Note for children of divorced/separated parents: The parent who brings the child is responsible for payment at the time of service. In the case of separated or divorced parents where one parent is court-ordered to pay for services, a copy of the court order (in its entirety) is required.
Dr. Gutow is a Licensed Psychologist and a Licensed School Psychologist with a broad background in teaching and providing assessment, therapy and consultation services.
